Library patrons who want to limit their borrowing to help avoid large fines.

Library Materials Available for Checkout
Print and media materials only.
(No Hotspots, Video Games or Kits)

3 Items at a time
Card must be renewed after 12 items have been check out

5 Items

Who Will Use This Card
Library patrons who:

  • Have no ID or proof of address
  • Dot not currently have a physical address
  • Are under age 14 & not accompanied by a legal guardian
  • Are over age 14 but have no proof of address
  • Want only public computer use

Library Card Terms of Service


I agree as a cardholder, parent or guardian, to accept full responsibility for all materials selected and borrowed on the library account, including any lost or damaged materials, or fines incurred. I agree to abide by all Library regulations.

I certify that the information provided on the application is true and correct to the best of my knowledge.

All patron records, including children’s, are confidential and as such are protected under Chapter 22.7 of the Iowa Code.

The Library does not restrict the use of library materials or use of the Internet for patrons of any age. Parents or guardians have the responsibility to guide their children's access to library materials.

  • Children under the age of 14 may receive:
    • Metro, Metro Lite, or Quick Card with a parent or guardian present
    • Quick Card without a parent or guardian present
  • Children over the age of 14 are eligible for all levels of library card without a parent or guardian present.

I agree to notify the Library if the card is lost or stolen. Failure to do so will result in my liability for all debts related to the account.

  • I will notify the Library of any change in contact information, including address, telephone number, or email.
  • Interlibrary loan (ILL) items must be returned to the Hiawatha Public Library. Failure to pick up ILL items will incur a fee. Quick Card users are not eligible to borrow items through ILL.
  • I agree to use multimedia items, including DVDs and CDs as intended and understand that the Library assumes no responsibility for equipment damage (CD, DVD player) that may result from use.
  • I understand that my Library record will be turned over to a collection agency if my bill reaches $40.00.
  • Courtesy notifications regarding due dates, overdue items and available holds will be sent by email or text if provided.
  • Accounts may be blocked in accordance with Hiawatha Public Library policies without notification. Due to service contract obligations only residents of Hiawatha, Cedar Rapids, Marion, Alburnett, Robins and Rural Linn Co. may use our downloadable e-book and audiobook services.

By accepting the library card, or as a parent/guardian permitting a child under the age of 14 to accept a Metro or Metro Lite card, I indicate that I have read and agreed to this policy.
